Learning Teams as an investigation tool
Bill Rigot, an accomplished engineer and veteran of the Navy's Nuclear Power Program and industry expert in human performance improvement and High Reliability Organizations joins us again as we shift from Old View vs. New View thinking about investigations. Following Dynamic Learning Activities (DLA) the presentation will discuss the use of Learning Teams to investigate events. The goal is to learn and improve rather than to blame and punish. The seven phases of conducting a Learning Team will be explored and case studies where learning teams were used and what they found that was different from previous investigations will be shared.
